FOSTER CARER AGREEMENT between {Name (First):78.3} {Name (Last):78.6} and Life of Pikelet:

Please be aware that some foster animals have unknown medical and behaviour history. We will always disclose all the information we are given.

By ticking the terms and conditions box and submitting this form to become a foster carer, you will be agreeing to the following:

- The the foster animal(s) will have a collar and ID tag on at ALL TIMES.
- That I will adhere to the instructions as noted in the FOSTER CARER MANUAL.
- That I will adhere to the Life of Pikelet Code of Conduct.
- That foster dogs are to be kept on a lead at ALL TIMES when not at the above listed property.
- That cats are never to be allowed outside unless in a completely secure cat run/enclosure.
- I further agree to assume all responsibility for the foster animal(s) in my care while on or off the premises.
- Each foster pet, for the period I am fostering the pet, is the sole property of Life of Pikelet. I have no proprietary interest in any foster pets.
- I understand it is important for my foster animal(s) to have a seperate area away from any household pets.
- To provide the animal(s) with loving care. This includes providing the animal(s) with adequate food, water and clean shelter which is an adequate space for the size, age and weight of the animal(s).
- That all food, goods and medical supplied that are given as donations to Life of Pikelet, are used only for foster animals under Life of Pikelet care.
- That if any emergency situation occurs with my foster animal(s) I will call Calley on 0404 038 369 or Lisa on 0430 808 094.
- That my own pets must be vaccinated and accept they may be at risk of contracting a contagious disease from contact with my foster animal(s). If this occurs, please notify Life of Pikelet, however medical costs may not be fully reimbursed.
- That if my foster animal(s) contract a disease/illness or has an injury it will be treated medically through Life of Pikelet approved vets using the booking number system as described in the FOSTER CARER MANUAL. Only costs approved will be covered by Life of Pikelet.
- If the animal(s) is taken to a non-Life of Pikelet vet, without prior approval, I may be liable for full costs.
- I, the foster carer(s), am responsible for the cost of any medical attention that result from an injury due to foster carer negligence.
- I will conduct myself in a manner that does not bring Life of Pikelet into disrepute, while performing fostering/volunteering duties.
- I fully understand and agree to assume all risks involved in any and all duties that I perform for Life of Pikelet in my volunteer capacity and I agree to hold Life of Pikelet harmless for any injury(s) that I might sustain during the course of my volunteer duties.
- That under no circumstances will I place a foster animal into another persons care or attempt to re-home the animal with prior written approval from Life of Pikelet.
- I will not under any circumstances leave any young children unsupervised with foster animal(s).
- To be a positive advocate and committed to the work and mission of Life of Pikelet.
- That I understand that Life of Pikelet advocates a cruelty-free lifestyle and as such only allows cruelty-free fundraisers.
